Children With Complex Health Needs

Community Children’s Nursing Service

The Community Children’s Nursing Service (CCNS) consists of a team manager, team leaders, children’s nurses and trained carers and is developing and appointing staff as required for the service.

The team provides respite nursing care for children and young people (0-19 years) with continuing health care needs in the community. This care will be given in partnership with
the family and other members of the multidisciplinary team.

Referral process

  • Key-worker or the person highlighting need for a service co-ordinates a multi-disciplinary continuing care assessment which should highlight whether child/young person meets the eligibility criteria for continuing health care.
  • Referrals are sent to the CCN team manager
  • A team leader will contact the family and complete a nursing assessment of need
  • Recommendation made for a service made to the continuing health care panel at Local Health Board
  • Child, family and key worker informed of decision made by LHB and arrangements made for commencement of service provision (as appropriate)
  • Respite care package will be reviewed on a regular basis.

2071 6617. If no one is available, messages can be left on the answering machine and a team
leader will get back to you.
